Size is A5, with 50 duplicate sets per book.  Both parts perforated for easy removal.
Each set is printed black on a two part NCR (no carbon required) 70gsm paper.
Top copy for the client – 2nd copy for your records.

Insert card for separating the next order.

Finished and glued along one edge for easy peel out of both copies.

Offset to one side for allowing hole punching if required.
